Job Title: Social Prescribing Link Worker x2        

Salary: £30,000 (12 months fixed term contract with 6-month probationary period and possible extension)

Hours: 37.5 hours per week

Place of work: South Liverpool

Responsible to: Services Manager

Purpose of post:

We’re looking for a passionate and empathic individual with qualifications and/or experience in community-led work to play a crucial role in supporting patients on a one to one and group basis to improve health and wellbeing outcomes. The successful candidate will be working collaboratively and creatively with patients, clinicians and wider stakeholders to generate better health outcomes for patients by co-creating and co-delivering a social prescribing service in the south of Liverpool.


Key responsibilities:

  • Undertake holistic 1-1 consultations with patients to address the social determinants of health
  • Work with patients to co-create personalised wellbeing plans by exploring what a good life looks like for them, and implementing these plans to improve health and wellbeing outcomes
  • Provide ‘hand-holding’ support to patients where required and accompany them to appointments where appropriate
  • Coordinate and deliver a range of educational and social inclusion activities and training for patients, staff and the public
  • Enable and support individuals to change their behaviour to improve their health and wellbeing and utilise community centred approaches to mobilise assets, strengths and develop capabilities
  • Actively participate in clinical Multi-Disciplinary Team meetings, Patient Participation Groups and attend wider networking opportunities; developing opportunities to work with others and supporting integrated working
  • Play a crucial role in the monitoring and attainment of key targets using a range of validated qualitative and quantitative methods
  • Recruit, induct, train and support volunteers
